Provide compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN), delivering skilled services directly in patients' homes within an assigned territory. This role involves managing a caseload of approximately 35 patient visits per day, utilizing Home Care Home Base (HCHB) for documentation and care coordination. Reliable transportation is essential, with mileage reimbursement typically provided.

Key Responsibilities:

• Conduct thorough nursing assessments and implement individualized care plans.
• Administer medications, perform wound care, manage IV therapy, and other clinical interventions as prescribed.
• Monitor patients health status and communicate changes promptly to supervising clinicians.
• Educate patients and caregivers on disease management, medication adherence, and self-care.
• Document all care activities and communications accurately using HCHB.
• Collaborate effectively with interdisciplinary teams to ensure adherence to care plans and regulatory standards.
• Uphold infection control protocols and patient safety at all times.
• Participate in case reviews, quality improvement initiatives, and ongoing training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• At least 1 year of experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB) software.
• Valid drivers license, reliable insured vehicle.
• Current BLS certification.
• Strong clinical judgment, organizational ability, and communication skills.
• Ability to work independently in a field-based setting.

Preferred:

• Experience with Medicare-certified home health agencies.
• Knowledge of home health compliance and documentation requirements.
• Exceptional interpersonal skills and a patient-focused care approach.

Work Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility based on patient needs.
