Schedule/Hours:

Qualifications:

  • Active LPN license
  • At least one year of recent home health experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR
  • Strong clinical assessment and documentation abilities
  • Reliable transportation and willingness to travel for home visits
  • Ability to manage caseload independently, demonstrating strong organizational skills

Responsibilities:

  • Deliver skilled nursing care according to individualized care plans in patients homes
  • Monitor, record, and communicate patient progress; update care plans to reflect changing needs
  • Administer medications and carry out treatments within regulatory guidelines
  • Provide patient and family education on disease prevention and effective care management
  • Maintain precise, timely documentation in HomeCare HomeBase (HCHB) EMR
  • Collaborate closely with RNs, therapists, and healthcare partners to support comprehensive continuity of care
