Schedule/Hours:
- Contract assignment (duration discussed during interview)
Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in home health nursing
- Proficiency with HomeCare HomeBase (HCHB) EMR system
- Solid clinical assessment and strong documentation abilities
- Dependable transportation for patient home visits
- Ability to independently manage a caseload while delivering high-quality patient care
Responsibilities:
- Deliver skilled nursing services to home-based patients as outlined in individualized care plans
- Monitor patient health, update care plans, and document progress using HomeCare HomeBase (HCHB)
- Administer prescribed medications and perform ordered treatments in accordance with state regulations
- Provide patient and family education on disease prevention and effective care management
- Collaborate closely with registered nurses, therapists, and other members of the care team to coordinate comprehensive services
