Deliver compassionate, high-quality care directly to patients where they are most comfortabletheir homes. As an experienced Home Health Licensed Practical Nurse, you'll play a vital role in empowering patients toward independence, education, and recovery while coordinating with families and an interdisciplinary healthcare team.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Occasional flexibility needed to accommodate patient needs

Key Qualifications:

• Active and unrestricted LPN license in the state of practice
• Minimum one year of experience in home health, hospice, or a similar community-based setting
• Proficiency with Home Care Home Base (HCHB) required
• Current BLS certification
• Valid drivers license, reliable transportation, and current vehicle insurance
• Able to work independently in a field-based role and manage a dynamic caseload
• Excellent clinical judgment, time management, and communication skills

Preferred Experience:

• Background with Medicare-certified home health agencies
• Knowledge of home health compliance and documentation standards
• Strong interpersonal skills and patient-centered approach

Primary Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized home health care
• Administer medications, perform wound care, and manage IV therapy or other clinical interventions per orders
• Monitor patient conditions and communicate findings to the RN Case Manager or physician
• Patient and caregiver education on disease processes, medication management, and self-care
• Document all care and communication in Home Care Home Base (HCHB)
• Collaborate with the broader care team, participate in case conferences, and maintain infection control standards
