Job Description
Deliver compassionate, skilled care in patients homes as a Licensed Practical Nurse. Home health nursing offers a unique and rewarding opportunity to build meaningful relationships, foster patient independence, and positively impact recoverywhile working independently in the field.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Responsibilities:
- Provide skilled nursing visits, assessing and implementing individualized plans based on each patients needs
- Administer prescribed treatments, including medication, wound care, and IV therapy
- Monitor patient conditions, promptly communicate changes to the care team, and collaborate on care planning
- Educate patients and caregivers on disease management, medications, and healthy habits to encourage recovery and independence
- Maintain meticulous documentation using Home Care Home Base (HCHB)
- Adhere to infection control protocols and maintain a safe care environment
- Attend case conferences, training, and contribute to quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license
- At least 1 year of experience in home health, hospice, or community-based care
- Proficiency using Home Care Home Base (HCHB) for patient records and documentation
- Current drivers license, reliable insured vehicle, willingness to travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong time management, clinical judgment, and communication skills
- Ability to work independently while managing a dynamic caseload
Preferred:
- Background with Medicare-certified home health agencies
- Thorough knowledge of compliance and regulatory requirements
- Excellent interpersonal and patient outreach abilities
