Deliver compassionate care to patients in their homes through this contract opportunity for a Licensed Practical Nurse (LPN) with experience in home health. The ideal candidate has a passion for working with geriatric populations and excels in using HomeCare HomeBase (HCHB) EMR to deliver seamless, high-quality care in a community setting.
Schedule:
- Contract assignment with potential for extension
Qualifications:
- Current, active LPN license
- At least one year of recent home health nursing experience
- Proficient in HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment and documentation abilities
- Reliable transportation for regular home visits
- Demonstrated ability to work independently and effectively manage a caseload
Key Responsibilities:
- Provide skilled nursing care in accordance with individualized care plans
- Monitor patient status and update care strategies as required
- Administer medications and perform medically necessary treatments per state guidelines
- Educate patients and their families on care processes and preventive strategies
- Maintain detailed, up-to-date documentation within HCHB EMR
- Collaborate with RNs, therapists, and interdisciplinary team members to ensure coordinated, comprehensive care
