Job Description

Deliver high-quality care and support to patients in their homes as a Licensed Practical Nurse specializing in home health. This full-time contract position focuses on providing patient-centered nursing services, helping individuals achieve greater independence and improved health outcomes. Youll manage a dynamic caseload, coordinate with healthcare professionals, and play a key role in patient education and advocacy.

Schedule:

  • MondayFriday, 8:00 AM5:00 PM
  • Occasional flexibility required to meet patient needs

Key Responsibilities:

  • Conduct skilled nursing visits and develop individualized patient care plans
  • Administer medications, perform wound care, and manage clinical interventions as ordered
  • Monitor and report patient condition changes to the RN Case Manager or physician
  • Educate patients and caregivers on health management and disease processes
  • Document all care and communication accurately using Home Care Home Base (HCHB)
  • Collaborate with the interdisciplinary team to coordinate care and maintain compliance
  • Adhere to infection control and patient safety standards at all times
  • Participate in case conferences, quality improvement initiatives, and ongoing training

Qualifications:

  • Active, unrestricted LPN license in the practicing state
  • At least one year of experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB)
  • Valid drivers license, reliable transportation, and current insurance
  • Current Basic Life Support (BLS) certification
  • Excellent clinical judgment, time management, and communication skills
  • Ability to work autonomously in the field and adapt to changing caseloads

Preferred Experience:

  • Previous work within Medicare-certified home health agencies
  • Deep understanding of compliance and documentation standards specific to home health
  • Outstanding interpersonal skills and a strong commitment to patient-centered care
