Deliver quality care and make a meaningful impact as a Home Health Licensed Practical Nurse (LPN). This contract role provides an outstanding opportunity to support patients in their homes by delivering skilled nursing services and empowering them through patient education and compassionate support.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
• Field-based, serving patients within an assigned territory
• Expected caseload: 2532 points per week (approximately 35 patient visits per day)

Key Qualifications:

• Active, unrestricted LPN license in state of practice
• At least 1 year of prior experience in home health, hospice, or similar community-based care
• Working knowledge of Home Care Home Base (HCHB) required
• Current BLS certification
• Valid driver's license and reliable personal transportation with current insurance
• Excellent time management, documentation, and communication skills
• Ability to work independently and manage a dynamic caseload
• Preferred: Experience with Medicare-certified home health agencies and strong understanding of home health compliance

Position Responsibilities:

• Deliver skilled nursing visits and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy
• Monitor and report changes in patient conditions to the RN Case Manager or physician
• Educate patients and caregivers on disease processes, medication, and self-care
• Ensure comprehensive, timely documentation in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary healthcare team for coordinated patient care
• Uphold infection control practices and maintain patient safety at all times
• Participate in case conferences, training, and quality improvement initiatives

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
