Job Description
Deliver high-quality, compassionate care as a Home Health Licensed Practical Nurse (LPN). This contract role is perfect for professionals who thrive working independently while supporting patients in the comfort of their own homes. As a field nurse, youll manage a dynamic caseload, promote patient independence, and collaborate closely with the interdisciplinary care team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state
- At least 1 year of home health, hospice, or related experience
- Proficiency with Home Care Home Base (HCHB) is required
- Valid drivers license and reliable vehicle with current insurance
- Current BLS certification
- Strong clinical judgment, communication, and time management skills
- Ability to work autonomously in a field-based setting
Preferred:
- Experience with Medicare-certified home health agencies
- Familiarity with compliance and home health documentation requirements
- Excellent interpersonal and patient education abilities
Core Responsibilities:
- Conduct skilled nursing visits and implement individualized care plans
- Administer medications, perform wound care, manage IV therapy, and perform other clinical interventions per physician orders
- Monitor, document, and promptly report changes in patient condition using HCHB
- Educate patients and caregivers on health management and self-care
- Coordinate seamlessly with RN Case Managers and the wider care team
- Maintain infection control and uphold patient safety during all interactions
- Participate in case conferences, training, and quality initiatives
