Deliver dedicated care to geriatric patients by utilizing your expertise as a Licensed Practical Nurse (LPN) in the home health setting. This contract opportunity calls for a compassionate, detail-oriented professional to deliver high-quality care and support patients independence in their own homes.
Schedule/Hours:
- Contract assignment with potential for extension
Qualifications:
- Active LPN license
- At least one year of recent experience in home health nursing
- Skilled in using HomeCare HomeBase (HCHB) EMR for documentation
- Excellent clinical assessment and patient documentation abilities
- Reliable transportation for in-home care visits
- Strong organizational skills and the ability to manage a caseload independently
Key Responsibilities:
- Deliver skilled nursing care to patients following tailored care plans
- Record and monitor patient progress, updating care plans as needed
- Administer medications and treatments in alignment with state guidelines
- Provide education and support to patients and family members on disease management and prevention
- Maintain precise and timely documentation in HomeCare HomeBase (HCHB) EMR
- Work collaboratively with registered nurses, therapists, and other care professionals to promote comprehensive patient care
