Job Description

Step into a rewarding opportunity as a Home Health Licensed Practical Nurse (LPN), where your skills make a profound impact every day. In this contract position, you'll deliver patient-centered nursing care to individuals in their homes, helping them achieve greater independence and improved health outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility based on patient needs

Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • At least one year of experience in home health, hospice, or similar community-based care
  • Proficiency with Home Care Home Base (HCHB) charting system
  • Valid drivers license, reliable transportation, and up-to-date auto insurance
  • Current BLS (Basic Life Support) certification
  • Strong clinical judgment, time management, and communication abilities
  • Ability to thrive independently while managing a dynamic, field-based caseload

Preferred Background:

  • Experience with Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-focused approach

Key Responsibilities:

  • Conduct skilled nursing visits (3-5 per day) to assess, plan, and implement patient care plans
  • Administer medications, wound care, and IV therapy as ordered
  • Monitor patient conditions and promptly report changes to care managers or physicians
  • Educate patients and caregivers on medications, disease management, and self-care techniques
  • Ensure accurate electronic documentation using HCHB
  • Collaborate closely with the interdisciplinary team to coordinate effective care
  • Uphold infection control practices and patient safety
  • Attend case conferences, ongoing training, and participate in quality improvement activities
