Seeking an experienced Home Health LPN to deliver compassionate, patient-focused care in a field-based role. This contract position is ideal for an autonomous nurse committed to empowering patients and ensuring regulatory compliance.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required for patient needs
Key Responsibilities:
- Conduct skilled nursing visits, assessing, planning, and implementing individualized care plans for patients in their homes
- Administer medications, wound care, IV therapies, and other clinical interventions as ordered
- Monitor and report changes in patient condition to the RN Case Manager or overseeing physician
- Educate patients and caregivers on health maintenance, medication, and disease process management
- Maintain accurate and timely documentation using Home Care Home Base (HCHB)
- Collaborate closely with an interdisciplinary team to coordinate and ensure continuity of care
- Uphold infection control standards and ensure patient safety at all times
- Participate in case conferences, quality improvement projects, and ongoing training as required
Required Qualifications:
- Active, unrestricted LPN license in Georgia
- Minimum of 1 year experience in home health, hospice, or a similar community-based setting
- Proficient in Home Care Home Base (HCHB) platform
- Valid drivers license with reliable transportation and current insurance
- Current BLS certification
- Excellent clinical judgment and communication skills
- Ability to work independently and manage a dynamic caseload
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Strong understanding of compliance standards and documentation in home health
- Outstanding interpersonal skills and a patient-centered approach
