Job Description

Seeking an experienced Home Health LPN to deliver compassionate, patient-focused care in a field-based role. This contract position is ideal for an autonomous nurse committed to empowering patients and ensuring regulatory compliance.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required for patient needs

Key Responsibilities:

  • Conduct skilled nursing visits, assessing, planning, and implementing individualized care plans for patients in their homes
  • Administer medications, wound care, IV therapies, and other clinical interventions as ordered
  • Monitor and report changes in patient condition to the RN Case Manager or overseeing physician
  • Educate patients and caregivers on health maintenance, medication, and disease process management
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB)
  • Collaborate closely with an interdisciplinary team to coordinate and ensure continuity of care
  • Uphold infection control standards and ensure patient safety at all times
  • Participate in case conferences, quality improvement projects, and ongoing training as required

Required Qualifications:

  • Active, unrestricted LPN license in Georgia
  • Minimum of 1 year experience in home health, hospice, or a similar community-based setting
  • Proficient in Home Care Home Base (HCHB) platform
  • Valid drivers license with reliable transportation and current insurance
  • Current BLS certification
  • Excellent clinical judgment and communication skills
  • Ability to work independently and manage a dynamic caseload

Preferred Qualifications:

  • Experience with Medicare-certified home health agencies
  • Strong understanding of compliance standards and documentation in home health
  • Outstanding interpersonal skills and a patient-centered approach
