Job Description
Deliver essential nursing care in patients homes by providing skilled, patient-focused services as a Home Health Licensed Practical Nurse (LPN). This contract opportunity is ideal for professionals who are passionate about supporting recovery and independence in a home setting while collaborating with a multidisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility to accommodate patient needs
Key Responsibilities:
- Conduct in-home skilled nursing assessments, develop and implement individualized care plans
- Administer medications, perform wound care, manage IV therapy, and carry out prescribed clinical interventions
- Monitor and document changes in patient conditions; report findings promptly to RN Case Manager or physician
- Deliver education to patients and caregivers regarding disease management, medication, and self-care
- Chart all care activities, interventions, and communication using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary team to coordinate and update patient care plans
- Uphold infection control procedures and patient safety standards
- Engage in case conferences, training, and ongoing quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license for the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficient in Home Care Home Base (HCHB) electronic charting
- Valid drivers license, reliable vehicle, and current auto insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, time management skills, and effective communication abilities
- Ability to operate independently in a field-based role and manage caseload efficiently
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and detailed documentation
