Job Description

Deliver essential care and support to patients where they feel most comfortabletheir own homesas a Home Health Licensed Practical Nurse (LPN) in the Florissant area. In this field-based, contract role, you will play a crucial part in ensuring positive health outcomes, education, and wellbeing for individuals managing acute or chronic health conditions outside a traditional facility setting.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to accommodate patient needs

Key Responsibilities:

  • Manage a dynamic caseload, conducting 35 patient visits per day (2532 points/week) within an assigned territory
  • Perform skilled nursing assessments, medication administration, wound care, IV therapy, and other ordered interventions
  • Provide patient and caregiver education on medication, self-care routines, and disease management
  • Meticulously document care and communications in the Home Care Home Base (HCHB) system to ensure compliance and service quality
  • Communicate patient status changes promptly to the RN Case Manager or physician, collaborating with the interdisciplinary team to adjust and coordinate care
  • Maintain infection control standards and overall patient safety
  • Join case conferences, quality improvements, and training sessions as needed

Required Qualifications:

  • Active, unrestricted LPN license in the state
  • At least one year of experience in home health, hospice, or similar community-based nursing care
  • Proficiency in Home Care Home Base (HCHB) charting system
  • Current BLS certification
  • Valid drivers license, reliable insured vehicle, and willingness to travel within territory
  • Excellent clinical judgment, time management, and communication skills
  • Ability to work autonomously in a field environment

Preferred Experience:

  • Work with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation standards
  • Strong interpersonal and patient-centered care skills
