Job Description
Deliver high-quality, compassionate nursing care to patients in their homes as an experienced Home Health Licensed Practical Nurse (LPN). In this field-based position, you will promote patient independence, provide education, and support recoveryall while ensuring consistent communication with patients, families, and the broader healthcare team. As an essential member of the home health staff, you will manage a caseload and be responsible for thorough and accurate documentation, compliant with regulatory and organizational standards.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Responsibilities:
- Perform skilled nursing visits, assessing, planning, and implementing individualized care for home health patients
- Administer medications, manage wound care, and deliver IV therapy or other prescribed clinical interventions
- Monitor and document changes in patient conditions; promptly report concerns to the RN Case Manager or physician
- Educate patients and caregivers about disease processes, medication administration, and self-care strategies
- Utilize Home Care Home Base (HCHB) for all documentation and communication
- Coordinate care within the interdisciplinary team and ensure alignment with prescribed care plans
- Follow infection control protocols and maintain a safe environment at all times
- Participate in case conferences, quality improvement programs, and required ongoing training
Required Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum of 1 year experience in a home health, hospice, or similar field-based role
- Proficiency in Home Care Home Base (HCHB)
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable vehicle, and current insurance
- Excellent clinical skills, communication, and time management
- Ability to work independently in a dynamic, field-based setting
Preferred Qualifications:
- Medicare-certified home health experience
- Deep understanding of home health compliance/documentation standards
- Strong interpersonal and patient-centered care approach
