Deliver exceptional patient care as a Home Health Licensed Practical Nurse (LPN) in a field-based role dedicated to supporting clients in their homes. This contract position requires a compassionate and detail-oriented professional ready to manage a dynamic caseload and ensure optimal outcomes for patients within their community. Your expertise in Home Care Home Base (HCHB) and commitment to meeting compliance standards will play a key role in providing quality-driven, individualized care.
Schedule/Hours:
- MondayFriday, 8:00 AM5:00 PM
- Occasional flexibility for patient needs
Key Responsibilities:
- Conduct skilled nursing visits (approx. 35 per day) to assess, implement, and revise personalized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as ordered
- Monitor and promptly report changes in patient status to the RN Case Manager or physician
- Educate patients and caregivers on disease processes, medication management, and self-care practices
- Accurately document all care and communication within Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary team to coordinate and update patient care
- Uphold infection control protocols and prioritize patient safety at all times
- Engage in case conferences, quality initiatives, and ongoing training
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or related community care
- Proficiency using HCHB
- Valid drivers license, reliable transportation, and current insurance
- Current BLS certification
- Excellent clinical assessment, time management, and communication skills
Preferred:
- Experience with Medicare-certified home health agencies
- Strong understanding of compliance and documentation
- Patient-focused interpersonal skills
