Provide compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) serving patients in their residences within a designated territory. This full-time contract opportunity offers Monday through Friday schedule from 8:00 AM to 5:00 PM, with occasional flexibility to meet patient needs. Manage a caseload of approximately 35 patient visits daily, averaging 2532 points per week, focusing on promoting patient independence and recovery through skilled assessments and interventions.

Key Responsibilities:

• Perform skilled nursing visits including medication administration, wound care, IV therapy, and other clinical procedures as ordered.
• Assess patient conditions, develop and implement personalized care plans, and monitor health status changes.
• Educate patients and caregivers on disease management, medication adherence, and self-care practices.
• Document all care and communications accurately in Home Care Home Base (HCHB) system.
• Collaborate with interdisciplinary teams to ensure coordinated, compliant patient care.
• Uphold infection control and patient safety standards.
• Participate in case conferences, quality improvement initiatives, and ongoing training.

Desired Experience and Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum one year of home health, hospice, or community-based nursing experience.
• Proficiency using Home Care Home Base (HCHB) electronic charting.
• Valid drivers license with reliable transportation and current insurance; mileage reimbursement provided.
• Current BLS certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to operate independently in the field and manage multiple patient visits efficiently.
• Preferred: Experience with Medicare-certified home health agencies and familiarity with home health compliance requirements.
