Experienced Licensed Practical Nurses with a focus on compassionate, high-quality home health care will find this contract opportunity rewarding and dynamic. This role emphasizes patient-centered nursing in a field-based setting, supporting the independence and recovery of individuals in their homes. Youll work closely with an interdisciplinary team, keeping patient communication and documentation at the forefront of daily operations.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Qualifications:
- Active, unrestricted LPN license in state of practice
- Minimum 1 year of home health, hospice, or similar community-based care experience
- Proficiency with Home Care Home Base (HCHB) charting system
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable vehicle, and current insurance
- Solid clinical judgment, organizational, and time management skills
- Ability to independently manage a dynamic caseload in a field environment
Preferred Experience:
- Work history in Medicare-certified home health agencies
- Familiarity with home health compliance, policy, and documentation
- Strong interpersonal skills and commitment to patient education
Core Responsibilities:
- Conduct skilled nursing visits, create and implement individualized care plans
- Administer medications, wound care, IV therapy, and clinical procedures per physician orders
- Stay alert to changes in patients conditions, promptly communicating findings
- Educate patients and caregivers about treatment, safety, and self-care practices
- Document all care and communication within the HCHB system
- Collaborate with the interdisciplinary healthcare team, attending case conferences and participating in quality improvement
