Job Description
Deliver exceptional care to patients in the comfort and familiarity of their own homes as a Home Health Licensed Practical Nurse (LPN). This contract, field-based position provides the opportunity to make a meaningful impact, promoting independence and recovery for home health clients while collaborating with a dedicated interdisciplinary team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
- Full-time contract assignment
- Field-based role covering assigned territory (35 home visits per day; 2532 points per week)
Responsibilities:
- Perform skilled nursing visits, develop and implement individualized care plans for each patient
- Administer medications, wound care, IV therapy, and other physician-ordered interventions
- Monitor patient conditions, promptly report changes, and communicate findings to the RN Case Manager or physician
- Provide patient and caregiver education on disease management, medications, and self-care
- Complete accurate charting and documentation in Home Care Home Base (HCHB)
- Ensure patient safety, infection control, and adherence to compliance standards
- Collaborate actively with the care team, attending case conferences and participating in training as required
Qualifications:
- Active, unrestricted LPN license in state of practice
- At least 1 year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) documentation system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS (Basic Life Support) certification
- Ability to work independently, with strong organizational, time management, and communication skills
Preferred:
- Experience with Medicare-certified home health agencies
- Skilled in understanding compliance and documentation standards
- Patient-centered, empathetic approach with excellent interpersonal skills
