Job Description
Step into a rewarding role as a Home Health Licensed Practical Nurse (LPN) and deliver patient-centered care directly in the comfort of clients homes. In this contract position, youll manage a caseload, promote patient independence, and ensure exceptional clinical quality while working closely with an interdisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required to meet patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year experience in home health, hospice, or similar community-focused setting
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license and reliable vehicle with current insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, communication, and time management abilities
- Comfortable working independently and managing a dynamic field caseload
Preferred Experience:
- Background in Medicare-certified home health agency
- In-depth knowledge of home health compliance and documentation standards
- Outstanding patient education and interpersonal skills
What Youll Do:
- Conduct skilled nursing assessments and implement individualized patient care plans
- Administer medications, perform wound care, and oversee IV therapy as prescribed
- Document all visits, care, and communications meticulously in HCHB
- Educate patients and caregivers on disease management and self-care
- Collaborate with the interdisciplinary team for optimal patient outcomes
- Maintain infection control protocols and uphold patient safety
- Attend case conferences, training sessions, and quality initiatives as needed
