Job Description
Deliver skilled nursing care in patients homes as a Licensed Practical Nurse (LPN) specializing in home health. This full-time contract position supports patient independence and recovery while ensuring compliance with home health standards. Work directly with patients and families to assess needs, provide clinical interventions, and educate on managing health conditions.
Key Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement individualized care for home health patients
- Administer medications, wound care, IV therapy, and other ordered treatments
- Monitor patient conditions, reporting changes to RN Case Managers or physicians promptly
- Use Home Care Home Base (HCHB) software to document assessments, treatments, and communications accurately
- Educate patients and caregivers on disease management, medication use, and self-care strategies
- Collaborate closely with interdisciplinary teams to ensure care plan adherence and regulatory compliance
- Maintain infection control standards and prioritize patient safety
- Participate in case conferences, quality improvement initiatives, and ongoing professional development
Qualifications:
- Current, unrestricted LPN license in the practicing state
- Minimum of 1 year home health, hospice, or community-based nursing experience
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable insured vehicle, and ability to travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to independently manage a dynamic caseload in a field-based role
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Compassionate and patient-centered communication approach
Schedule & Location:
- Monday to Friday, 8:00 AM to 5:00 PM with occasional schedule flexibility based on patient needs
- Field-based visits within a designated territory in Evans, GA
- Approximately 35 patient visits daily, managing a caseload of 2532 points weekly
