Provide skilled nursing care to patients in their homes as a Licensed Practical Nurse specializing in Home Health. This role requires managing a caseload, delivering patient-centered care, and maintaining thorough and compliant documentation using Home Care Home Base (HCHB). The LPN will collaborate closely with healthcare team members and focus on promoting patient independence and education.

Location: Your work will be based in the Evans, GA area, providing care to patients within an assigned territory. The role is field-based, requiring reliable transportation for travel to patient homes. Mileage reimbursement is typically provided.

Key Responsibilities:

• Conduct skilled nursing visits to assess, develop, and implement individualized care plans.
• Administer medications, perform wound care, manage IV therapy, and other clinical interventions as ordered.
• Monitor patient conditions and communicate changes promptly to appropriate medical staff.
• Educate patients and caregivers on disease management, medication adherence, and self-care skills.
• Document all care activities and communications accurately in HCHB.
• Collaborate with interdisciplinary teams to ensure care coordination and regulatory compliance.
• Uphold infection control standards and maintain patient safety.
• Participate in case conferences, quality improvement initiatives, and ongoing training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum one year of experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB) charting system.
• Valid drivers license, reliable vehicle, and current insurance.
• Current BLS certification.
• Strong clinical judgment, effective time management, and communication skills.
• Ability to work independently while managing a dynamic patient caseload.

Preferred:

• Experience with Medicare-certified home health agencies.
• Familiarity with home health compliance and documentation requirements.
• Patient-centered attitude and excellent interpersonal skills.

Schedule: Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility depending on patient needs. Expect approximately 3-5 patient visits per day, totaling 25-32 workload points per week.
