Experienced Licensed Practical Nurse (LPN) Home Health Care
Provide compassionate, patient-focused nursing care in home settings, supporting individuals recovery, independence, and education. This full-time, contract role involves managing a caseload of home health patients, performing skilled nursing tasks, and maintaining detailed documentation in compliance with regulations through Home Care Home Base (HCHB).

Schedule
- Monday Friday
- 8:00 AM 5:00 PM with occasional flexibility for patient needs
- Approximately 2532 points/week (35 patient visits per day)
Key Responsibilities
- Conduct skilled nursing visits to assess and implement patient-specific care plans
- Administer medications, perform wound care, and manage IV therapy as ordered
- Monitor patient condition changes and communicate promptly with the care team
- Educate patients and caregivers on health management and disease processes
- Document all care and communication accurately using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare teams to ensure coordinated care
- Maintain infection control and patient safety standards
- Participate in case conferences and quality improvement initiatives
Qualifications & Experience
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) required
- Valid drivers license with reliable transportation and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based setting
Preferred
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Exceptional interpersonal skills with a patient-centered care approach
