Job Description
Deliver in-home nursing care while supporting recovery and patient independence in a vital, field-based role. This full-time contract opportunity centers on providing skilled clinical services to home health patients across your assigned territory in Englewood, CO. Youll play a key part in promoting holistic wellness, educating patients and caregivers, and collaborating with an interdisciplinary care team.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
- Field-based; approximately 35 patient visits per day (2532 points weekly)
Responsibilities:
- Conduct home visits to assess, plan, and carry out individualized care for patients
- Perform medication administration, wound care, IV therapy, and other clinical interventions as ordered
- Monitor and report changes in patient condition to RN Case Manager or physician
- Educate patients and their families on care techniques and medication management
- Accurately document care and communication using Home Care Home Base (HCHB)
- Coordinate care in collaboration with the interdisciplinary healthcare team
- Uphold infection control protocols and maintain patient safety at all times
- Participate in trainings, case conferences, and quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or a similar setting
- Proficient with Home Care Home Base (HCHB)
- Current BLS certification
- Valid drivers license, reliable transportation, and up-to-date insurance
- Ability to manage a dynamic caseload independently
- Strong clinical judgment, organization, and communication abilities
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation
- Patient-focused mindset with excellent interpersonal skills
