Job Description

Delivering skilled, compassionate care to patients in the comfort of their own homes, this field-based Licensed Practical Nurse opportunity offers a rewarding way to support recovery and independence. Youll apply your Home Health expertise, work independently, and provide vital support to patients and their families by ensuring high-quality, patient-centered care.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

What You'll Do:

  • Conduct skilled nursing visits and develop individualized care plans tailored to home health patients
  • Administer medications, perform wound care, and manage IV therapy or other physician-ordered interventions
  • Monitor and report any changes in patient conditions to the RN Case Manager or physician
  • Provide education to patients and caregivers regarding illness management, medications, and self-care strategies
  • Complete thorough documentation of assessments and interventions using the Home Care Home Base (HCHB) charting system
  • Collaborate with a multidisciplinary team to ensure complete and compliant care coordination
  • Maintain strict adherence to infection control standards and promote patient safety at every interaction
  • Attend case conferences, quality improvement sessions, and training as needed

Qualifications:

  • Active, unrestricted LPN license in Illinois
  • Minimum one year of experience in home health, hospice, or a similar environment
  • Home Care Home Base (HCHB) proficiency required
  • Current BLS certification
  • Valid drivers license, reliable vehicle, and proof of current auto insurance
  • Strong clinical judgment, excellent time management, and communication skills
  • Confident working independently in a field-based, community care setting

Preferred:

  • Background in Medicare-certified home health
  • Strong understanding of home health compliance and documentation standards
  • Excellent interpersonal skills, dedicated to patient-centered care
