Deliver meaningful patient care as a Home Health Licensed Practical Nurse (LPN) in a field-based role where clinical expertise and compassion go hand-in-hand. This contract position offers the opportunity to engage one-on-one with patients, promoting their independence and supporting recovery in the comfort of their homes.

Schedule/Hours:

• Monday Friday, 8:00 AM to 5:00 PM
• Occasional flexibility required based on patient needs

Key Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least 1 year of home health, hospice, or community-based care experience
• Proficiency with Home Care Home Base (HCHB) electronic charting system
• Valid drivers license, reliable transportation, and current insurance
• Current BLS certification
• Strong clinical judgment, organization, and communication abilities
• Ability to work independently in a dynamic field environment
• Preferred: Medicare-certified home health experience and strong compliance/documentation skills

Role Responsibilities:

• Manage a caseload and conduct 35 patient visits daily within the assigned territory (2532 points per week)
• Assess, plan, and implement individualized care for home health patients
• Administer medications, provide wound care, and perform clinical interventions as ordered
• Monitor and report patient condition changes to the RN Case Manager or physician
• Educate patients/caregivers on self-care, medication management, and disease processes
• Document all assessments, care, and communications accurately using HCHB
• Collaborate with the interdisciplinary team and ensure compliance with care plans
• Uphold infection control and patient safety standards
• Participate in case conferences, quality improvement, and required trainings

$1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
