Job Description
Deliver essential patient care in a home-based setting as a Licensed Practical Nurse. This contract position focuses on supporting individuals in their recovery and daily health management within their homes, emphasizing patient education, safety, and independence. As part of your daily routine, youll manage a caseload and coordinate closely with the broader healthcare team, ensuring every individual receives high-quality, compassionate care tailored to their needs.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key responsibilities include:
- Completing skilled nursing home visits, developing and implementing individualized care plans
- Administering medications, wound care, and managing IV or other clinical treatments as ordered
- Monitoring and reporting patient condition changes to the RN Case Manager or physician
- Providing education to patients and caregivers on disease management and self-care techniques
- Maintaining diligent, timely documentation in Home Care Home Base (HCHB)
- Ensuring all care complies with applicable standards and protocols, including infection control
- Participating in interdisciplinary meetings, case conferences, and training sessions
- Maintaining accurate and up-to-date patient records, ensuring regulatory compliance throughout
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or related community-based care
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and valid auto insurance
- Current BLS certification
- Solid clinical judgment, organization, and communication skills
- Ability to work independently and manage a dynamic, field-based caseload
Preferred:
- Background in Medicare-certified home health agency environments
- In-depth knowledge of home health compliance and documentation standards
- Strong interpersonal skills
