Job Description

Experienced Licensed Practical Nurse (LPN) opportunity available in home health, delivering compassionate, patient-focused nursing care in a field-based setting. This role emphasizes promoting patient independence and recovery through skilled nursing visits within the patient's home environment.

Location:
Position is based in Duluth, GA, an inviting community known for its blend of suburban comfort and access to vibrant cultural amenities. Duluth offers a welcoming atmosphere, with parks, shopping centers, and dining options ideal for balancing work and personal life.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional schedule flexibility to meet patient needs
  • Expect 35 patient visits per day, managing 2532 workload points weekly

Responsibilities:

  • Conduct thorough skilled nursing assessments and implement individualized care plans
  • Administer medications, manage wound care, IV therapy, and other clinical treatments
  • Monitor and report patient condition changes promptly to the RN Case Manager or physician
  • Educate patients and caregivers on medication management and self-care techniques
  • Complete accurate and timely documentation using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary healthcare teams to coordinate compliant patient care
  • Maintain strict infection control standards and ensure patient safety
  • Participate in case conferences, quality improvement, and ongoing training

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of experience in home health, hospice, or comparable community care
  • Proficiency with Home Care Home Base (HCHB) documentation system
  • Valid drivers license and reliable insured vehicle
  • Current BLS certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently in a field-based role managing diverse patient needs

Preferred:

  • Experience with Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-centered care approach
