Job Description
Make a meaningful impact as a compassionate Home Health Licensed Practical Nurse (LPN) providing direct, patient-centered care in individuals homes. This contract opportunity offers a rewarding chance to engage with patients where they are most comfortable, helping support independence and recovery.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional flexibility required to accommodate patient needs
- Field-based, covering an assigned territory (travel required, mileage reimbursement typically provided)
- Anticipate 2532 points per week (about 35 patient visits per day)
Key Responsibilities:
- Conduct skilled nursing assessments and implement individualized patient care plans during home visits
- Administer medications, wound care, IV therapies, and other prescribed clinical interventions
- Monitor and report changes in patient conditions to the RN Case Manager or physician
- Educate patients and caregivers regarding disease management, medication administration, and self-care
- Accurately document care and communications using Home Care Home Base (HCHB)
- Coordinate with the interdisciplinary team, maintaining compliance with care plans and home health regulations
- Uphold infection control and patient safety standards at all times
- Participate in required trainings, quality improvement activities, and case conferences as needed
Required Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least one year of experience in home health, hospice, or similar community-based care
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable vehicle, current insurance
- Current BLS certification
- Excellent time-management and independent work skills
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Strong knowledge of documentation and compliance requirements
- Demonstrated patient-focused communication and interpersonal skills
