Provide compassionate, skilled nursing care as a Home Health Licensed Practical Nurse (LPN) dedicated to supporting patients in their homes. This role emphasizes promoting patient independence, education, and recovery while maintaining clear communication with patients, families, and the interdisciplinary healthcare team. You will manage a caseload of patients, ensuring all documentation and care plans comply with regulatory and organizational standards.

Responsibilities
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as prescribed
- Monitor patient condition changes and communicate promptly with RN Case Managers or physicians
- Educate patients and caregivers on disease processes, medication management, and self-care techniques
- Accurately document all visits and communication using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary healthcare team to coordinate care and ensure plan compliance
- Maintain infection control and patient safety standards
- Participate in case conferences, quality initiatives, and mandatory training
Qualifications
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license and reliable insured vehicle
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently managing a dynamic caseload
Preferred
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused approach
Schedule & Contract
- Full-time contract position
- Monday Friday, 8:00 AM 5:00 PM with occasional flexibility
- Manage approximately 35 patient visits per day (2532 points weekly)
