• Reliable transportation and valid drivers license.

Responsibilities:

• Deliver professional nursing services to patients in their homes according to individualized care plans.
• Monitor, record, and report patient conditions, treatments, and progress to supervising RNs or physicians.
• Administer medications and treatments as prescribed.
• Educate patients and families about care procedures and health management.
• Coordinate with multidisciplinary care teams to ensure continuity and quality of care.
