Key Responsibilities:
- Deliver high-quality nursing care to patients in their residences, following detailed care plans and physician orders
- Perform patient assessments, observe symptoms and report changes in condition to clinical supervisors
- Administer prescribed medications, treatments, and injections as directed by healthcare providers
- Provide education to patients and families on treatment plans and relevant health conditions
- Collaborate with interdisciplinary teams to support comprehensive, individualized patient care
- Ensure accurate documentation in electronic health records, maintaining compliance with all healthcare regulations and policies
- Travel within the assigned service area to fulfill patient visits as scheduled
Desired Skills & Qualifications:
- Current LPN license in the state of Georgia
- Previous experience in Home Health or a strongly related environment is highly preferred
- Familiarity with nursing best practices, medication administration, and wound care
- Strong clinical assessment and communication skills
- Comfort working independently and managing a mobile work schedule
- Valid drivers license and reliable transportation
