Job Description

Deliver compassionate, patient-centered nursing care in the comfort of patients' homes as a Home Health Licensed Practical Nurse (LPN). This contract opportunity is ideal for an experienced clinician who thrives in a field-based environment and values autonomy, high-quality care, and collaborative teamwork.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional schedule flexibility to meet patient needs

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or related home-based care
  • Proficiency with Home Care Home Base (HCHB) electronic charting
  • Valid drivers license, reliable vehicle, and current insurance
  • BLS (Basic Life Support) certification
  • Sound clinical skills, strong communication, and excellent time management
  • Ability to work independently in a field setting and efficiently manage a caseload

Preferred Attributes:

  • Experience in Medicare-certified home health agencies
  • Familiarity with home health compliance and documentation requirements
  • Excellent interpersonal skills and a dedication to patient education

Responsibilities:

  • Conduct skilled patient visits and provide comprehensive assessment and individualized care
  • Administer medications, wound care, and clinical interventions as prescribed
  • Monitor patient conditions and communicate changes to the care team
  • Educate patients and caregivers on illness management and self-care
  • Accurately chart all patient encounters and communications using HCHB
  • Collaborate with interdisciplinary team members to coordinate care and participate in quality initiatives
  • Uphold infection control and safety protocols during all interactions

$1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
