Providing compassionate, skilled nursing care in the homes of patients, this opportunity is ideal for Licensed Practical Nurses (LPNs) experienced in home health settings. Key Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement individualized care plans in patients homes.
- Administer medications, wound care, and clinical treatments such as IV therapy under physician orders.
- Monitor patients conditions and communicate changes promptly to RNs or physicians.
- Educate patients and caregivers on disease management, medication adherence, and self-care techniques.
- Maintain accurate documentation using Home Care Home Base (HCHB) charting system.
- Collaborate with interdisciplinary healthcare teams to ensure coordinated care and compliance.
- Uphold infection control protocols and patient safety standards.
- Participate in case conferences, quality improvement, and required continuing education.
Qualifications and Desired Experience:
- Active, unrestricted LPN license valid in the practicing state.
- Minimum of 1 year experience in home health, hospice, or community-based care.
- Proficient use of Home Care Home Base (HCHB) electronic documentation.
- Valid drivers license, reliable transportation, and current insurance.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, communication, and time management skills.
- Able to work independently managing a dynamic caseload in a field environment.
- Preferred: Experience with Medicare-certified home health agencies and knowledge of home health compliance standards.
Schedule & Contract Details:
- Full-time contract role.
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility.
- Approximately 35 patient visits daily (2532 points weekly).
- Mileage reimbursement generally provided.
