Key Responsibilities:

  • Perform skilled nursing visits to assess, plan, and implement individualized care plans in patients homes.
  • Administer medications, wound care, and clinical treatments such as IV therapy under physician orders.
  • Monitor patients conditions and communicate changes promptly to RNs or physicians.
  • Educate patients and caregivers on disease management, medication adherence, and self-care techniques.
  • Maintain accurate documentation using Home Care Home Base (HCHB) charting system.
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated care and compliance.
  • Uphold infection control protocols and patient safety standards.
  • Participate in case conferences, quality improvement, and required continuing education.

Qualifications and Desired Experience:

  • Active, unrestricted LPN license valid in the practicing state.
  • Minimum of 1 year experience in home health, hospice, or community-based care.
  • Proficient use of Home Care Home Base (HCHB) electronic documentation.
  • Valid drivers license, reliable transportation, and current insurance.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical judgment, communication, and time management skills.
  • Able to work independently managing a dynamic caseload in a field environment.
  • Preferred: Experience with Medicare-certified home health agencies and knowledge of home health compliance standards.

Schedule & Contract Details:

  • Full-time contract role.
  • Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility.
  • Approximately 35 patient visits daily (2532 points weekly).
  • Mileage reimbursement generally provided.
