Job Description

Provide essential care and make a difference in the community as a Licensed Practical Nurse specializing in home health. In this vital role, you will deliver skilled nursing services directly to patients in their homes, supporting recovery, independence, and education across diverse patient populations. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to accommodate patient needs

Key Responsibilities:

  • Complete skilled nursing assessments and implement individualized care plans for home health patients
  • Administer medications, perform wound care, manage IV therapy, and assist with other prescribed treatments in alignment with care plans
  • Closely monitor patient condition changes; communicate promptly with the RN Case Manager and physician as necessary
  • Educate patients and caregivers on managing health conditions, medications, and self-care
  • Ensure accurate and timely documentation in Home Care Home Base (HCHB)
  • Collaborate with an interdisciplinary team to support optimal patient outcomes and compliance
  • Maintain rigorous infection control and safety protocols during care delivery
  • Participate in case conferences, ongoing training, and quality improvement activities

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of one years experience in home health, hospice, or community-based care
  • Proficiency with Home Care Home Base (HCHB) electronic charting system
  • Valid drivers license, reliable transportation, and current automobile insurance
  • Current BLS (Basic Life Support) certification
  • Effective communication, clinical decision-making, and time management skills
  • Ability to operate independently in the field and handle a dynamic patient caseload

Preferred:

  • Prior experience with Medicare-certified home health agencies
  • Familiarity with home health compliance requirements
