Job Description
Deliver compassionate, high-quality care as a Licensed Practical Nurse focused on home health services. In this rewarding field-based position, youll ensure patients receive exceptional nursing care in their homes, fostering independence, recovery, and well-being. Your work will directly impact patients and families by providing skilled clinical interventions, education, and ongoing support across an assigned caseload.
Schedule:
- Full-time contract
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility based on patient needs)
Key Responsibilities:
- Perform skilled nursing visits, including comprehensive assessments and individualized care plans
- Administer medications, wound care, IV therapy, and other clinical treatments as ordered
- Observe and promptly communicate patient status changes to the RN Case Manager or physician
- Educate patients and caregivers on health management, medications, and self-care strategies
- Accurately document all care and communications in Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary healthcare team to ensure effective implementation of care plans
- Adhere to infection control protocols and maintain the highest patient safety standards
- Participate in case conferences, required training, and quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) is mandatory
- Valid drivers license, reliable transportation, and current vehicle insurance
- BLS (Basic Life Support) certification
- Strong clinical decision-making, organization, and communication skills
- Ability to work independently while managing a varied caseload
- Preferred: Experience with Medicare-certified home health agencies and strong home health compliance background

Compensation for this position ranges from $17.45 to $30.00. Eligible candidates may also receive healthcare benefits, housing, and meals/incidentals. Full details of our healthcare benefits plan can be found here. This posting is open for 60 days after the posting date.
