Job Description
Provide compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) in a supportive, field-based role located in Derry, NH. This full-time contract position requires managing a caseload of patients within their homes and delivering care aligned with regulatory standards using the Home Care Home Base (HCHB) charting system.
Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans for home health patients.
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions as ordered.
- Monitor patient condition changes and promptly report findings to the RN Case Manager or physician.
- Educate patients and caregivers on disease processes, medication management, and self-care techniques.
- Accurately document all assessments, care provided, and communications using HCHB.
- Collaborate with the interdisciplinary healthcare team to coordinate patient care and ensure compliance.
- Maintain infection control standards and ensure patient safety.
- Participate in case conferences, quality improvement activities, and required training.
Qualifications:
- Active and unrestricted LPN license in the state of practice.
- Minimum 1 year of experience in home health, hospice, or similar community-based care settings.
- Proficiency in Home Care Home Base (HCHB) software.
- Valid drivers license, reliable vehicle with current insurance.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, time management, and communication skills.
- Ability to independently manage a dynamic caseload and work in a field-based environment.
Preferred Experience:
- Prior work with Medicare-certified home health agencies.
- Familiarity with home health compliance and documentation standards.
- Excellent interpersonal skills with a patient-centered approach.
Schedule & Workload:
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs.
- Approximately 35 patient visits per day, totaling 2532 points weekly.
- Must have reliable transportation; mileage reimbursement is typically provided.
