Job Description
Deliver compassionate care in a dynamic, patient-centered environment as a Licensed Practical Nurse specializing in home health. This full-time contract opportunity focuses on providing skilled nursing services to individuals in their homes, supporting independence and recovery for a diverse caseload.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Key Responsibilities:
- Complete skilled nursing assessments and develop individualized care plans for homebound patients
- Administer medications, perform wound care, and manage IV therapy or clinical interventions as ordered
- Closely monitor patient condition changes; promptly communicate findings to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication administration, and self-care
- Accurately document care and assessments using Home Care Home Base (HCHB)
- Collaborate regularly with interdisciplinary healthcare teams to ensure seamless transition and compliance within the plan of care
- Maintain strong infection control and safety standards in all patient interactions
- Attend case conferences, participate in quality improvement efforts, and complete required training
Required Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year of experience in home health, hospice, or a community-based care setting
- Demonstrated proficiency with Home Care Home Base (HCHB) electronic medical record system
- Valid drivers license, reliable vehicle, and current auto insurance
- Current BLS (Basic Life Support) certification
- Strong clinical, communication, and time management skills
- Ability to work independently and manage a dynamic, field-based caseload
Preferred Attributes:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation requirements
