Job Description
Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in a field-based setting. This full-time contract role involves managing a caseload of patients within an assigned territory, delivering skilled nursing visits, and collaborating closely with an interdisciplinary team to ensure high-quality home health care.
Key Responsibilities:
- Conduct thorough skilled nursing assessments, develop, and implement individualized care plans for patients in their homes.
- Administer medications, perform wound care, manage IV therapy, and other clinical interventions as prescribed.
- Monitor patient conditions, promptly communicating changes to the RN Case Manager or physician.
- Educate patients and caregivers on disease management, medication adherence, and self-care practices.
- Accurately document all nursing activities and communications using Home Care Home Base (HCHB) software.
- Maintain strict infection control protocols and patient safety standards.
- Participate in case conferences, quality improvement initiatives, and mandatory training.
Qualifications and Desired Experience:
- Active and unrestricted LPN license valid in the state of practice.
- Minimum of one year of experience in home health, hospice, or community-based care settings.
- Proficiency in Home Care Home Base (HCHB) documentation system.
- Valid drivers license, reliable insured vehicle, and willingness to travel within the assigned territory.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, time management, and self-directed work capabilities.
- Prior Medicare-certified home health agency experience and knowledge of home health compliance standards preferred.
Schedule and Workload:
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility based on patient needs.
- Approximately 2532 points weekly, equivalent to 35 patient visits daily.
