Desired Qualifications and Experience:

• Current and valid LPN license
• Home health care experience strongly preferred
• Demonstrated proficiency in clinical skills necessary for home health visits
• Excellent communication, organizational, and time-management abilities
• Ability to work independently while collaborating closely with a multidisciplinary healthcare team
• Current CPR/BLS certification

Key Responsibilities:

• Deliver high-quality skilled nursing care for patients in their homes
• Perform assessments, monitor patient health, and document care in compliance with established standards
• Administer prescribed medications and treatments
• Educate patients and their families on managing conditions and medications
• Communicate patient status to supervising Registered Nurses and physicians
• Ensure timely and accurate charting in electronic health records
• Uphold company policies and federal/state regulations regarding patient care and privacy
