Schedule/Hours:

Qualifications and Experience:
- Current and unencumbered LPN license
- Minimum of one year of recent experience as a Licensed Practical Nurse in a healthcare setting
- Prior experience in home health nursing strongly preferred
- Strong clinical assessment, problem-solving, and documentation skills
- Ability to work independently, manage time efficiently, and handle a varied caseload
- Excellent communication and interpersonal skills with patients, families, and other healthcare professionals
- Valid drivers license and reliable transportation
Key Responsibilities:
- Provide direct nursing care to patients in their homes according to individualized care plans
- Perform assessments, monitor patient conditions, and report significant changes to the RN or physician
- Administer medications, treatments, and wound care as ordered
- Educate patients and families about care procedures, self-care, and medication management
- Collaborate with interdisciplinary healthcare team members to ensure high-quality patient-centered care
- Maintain accurate and timely documentation in accordance with regulations and policies
