An experienced Licensed Practical Nurse (LPN) is needed for a contract position specializing in home health care. This role is perfect for a dedicated LPN passionate about delivering high-quality nursing care to geriatric patients in their own homes. Prior experience in home health and proficiency with HomeCare HomeBase (HCHB) electronic medical records are essential for success in this position.
Responsibilities include providing skilled nursing care according to individualized care plans, monitoring and documenting patient progress, administering medications and treatments within state guidelines, and educating patients and families on disease management and prevention. The nurse will also maintain accurate electronic records using HCHB EMR and collaborate closely with registered nurses, therapists, and other healthcare professionals to ensure comprehensive patient care.
Qualifications:
- Active LPN license
- At least one year of recent home health nursing experience
- Proficiency with HomeCare HomeBase (HCHB) EMR system
- Strong clinical assessment and documentation capabilities
- Reliable transportation for patient home visits
- Ability to work independently while managing a caseload efficiently
