Job Description

An experienced Licensed Practical Nurse (LPN) is needed for a contract position specializing in home health care. The nurse will also maintain accurate electronic records using HCHB EMR and collaborate closely with registered nurses, therapists, and other healthcare professionals to ensure comprehensive patient care.

Qualifications:

  • Active LPN license
  • At least one year of recent home health nursing experience
  • Proficiency with HomeCare HomeBase (HCHB) EMR system
  • Strong clinical assessment and documentation capabilities
  • Reliable transportation for patient home visits
