A dedicated Licensed Practical Nurse (LPN) with home health experience is sought for a contract position focused on providing skilled nursing care to geriatric patients in their homes. This role requires proficiency in HomeCare HomeBase (HCHB) EMR and a commitment to high-quality, compassionate patient care.

Responsibilities:
- Deliver skilled nursing care according to individualized care plans

Qualifications:
- Active LPN license

- Strong clinical assessment and documentation ability
- Reliable transportation for conducting home visits
- Self-motivated with the ability to independently manage a caseload
