Job Description

Provide expert patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) within a full-time contract role based in Dallas, OR. This field-based nursing position involves delivering compassionate, skilled care directly in patients homes, supporting their independence, recovery, and education. Managing your caseload effectively with a focus on compliance and collaboration is essential in this role.

Schedule & Workload:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility required)
  • Approximately 35 patient visits daily (2532 points per week)
  • Reliable transportation required for travel within assigned territory

Key Responsibilities:

  • Conduct skilled nursing visits to assess, plan, and implement individualized care plans
  • Administer medications, wound care, IV therapy, and other clinical interventions
  • Monitor patient condition changes and report promptly to RN Case Manager or physician
  • Educate patients and caregivers on medication management, disease processes, and self-care
  • Document all care and communication accurately using Home Care Home Base (HCHB) system
  • Collaborate with interdisciplinary teams to ensure care plan compliance
  • Uphold infection control and patient safety standards
  • Participate in case conferences, quality improvement, and trainings

Qualifications:

  • Active and unrestricted LPN license in Oregon
  • Minimum 1 year of home health, hospice, or community-based nursing experience
  • Proficient in Home Care Home Base (HCHB) charting system
  • Valid drivers license with reliable vehicle and insurance
  • Current BLS certification
  • Strong clinical judgment, communication, and time management skills
  • Ability to independently manage a dynamic caseload

Preferred Experience:

  • Familiarity with Medicare-certified home health agencies
  • Thorough knowledge of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-focused approach
