Job Description
Provide expert patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) within a full-time contract role based in Dallas, OR. This field-based nursing position involves delivering compassionate, skilled care directly in patients homes, supporting their independence, recovery, and education. Managing your caseload effectively with a focus on compliance and collaboration is essential in this role.
Schedule & Workload:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility required)
- Approximately 35 patient visits daily (2532 points per week)
- Reliable transportation required for travel within assigned territory
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions
- Monitor patient condition changes and report promptly to RN Case Manager or physician
- Educate patients and caregivers on medication management, disease processes, and self-care
- Document all care and communication accurately using Home Care Home Base (HCHB) system
- Collaborate with interdisciplinary teams to ensure care plan compliance
- Uphold infection control and patient safety standards
- Participate in case conferences, quality improvement, and trainings
Qualifications:
- Active and unrestricted LPN license in Oregon
- Minimum 1 year of home health, hospice, or community-based nursing experience
- Proficient in Home Care Home Base (HCHB) charting system
- Valid drivers license with reliable vehicle and insurance
- Current BLS certification
- Strong clinical judgment, communication, and time management skills
- Ability to independently manage a dynamic caseload
Preferred Experience:
- Familiarity with Medicare-certified home health agencies
- Thorough knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused approach
