An exciting opportunity is available for a skilled and compassionate Licensed Practical Nurse (LPN) with expertise in home health care. In this crucial role, you will deliver direct, patient-centered nursing services to clients in their homes, focusing on patient recovery, safety, and independence. Nurses in this field-based position manage a caseload throughout the week, ensuring top-quality care and contributing to each individual's well-being.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Flexibility for occasional patient needs
Qualifications:
- Current, unrestricted LPN license in the state of practice
- Minimum of 1 year of recent experience in home health, hospice, or community-based care
- Proficient with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable transportation, and current auto insurance
- Current Basic Life Support (BLS) certification
- Strong clinical decision-making, organization, and communication abilities
- Ability to work independently while managing a fluctuating caseload
Preferred experience:
- Background with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation best practices
- Demonstrated compassionate and patient-focused approach
Key responsibilities:
- Conduct home visits, assess patient needs, and execute individualized care plans
- Administer medications, provide wound care, and perform prescribed clinical interventions
- Monitor and document patient progress accurately in Home Care Home Base (HCHB)
- Educate patients and caregivers on treatment plans and self-care techniques
- Maintain robust infection control and promote a safe home environment
- Collaborate closely with interdisciplinary care team members
