Job Description

A skilled Home Health Licensed Practical Nurse (LPN) is needed to deliver high-quality, compassionate nursing care to patients in their homes. This field-based role centers on patient independence, education, and recovery, while ensuring excellent communication with individuals, families, and the interdisciplinary healthcare team. The nurse manages a caseload and upholds documentation standards using Home Care Home Base (HCHB), meeting all regulatory and organizational guidelines.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility for patient needs

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least one year of experience in home health, hospice, or a similar community-based setting
  • Proficiency with Home Care Home Base (HCHB)
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Strong clinical judgment, organization, and communication skills
  • Ability to self-manage and adapt to a dynamic caseload in a field-based environment

Preferred Experience:

  • Familiarity with Medicare-certified home health agencies
  • Strong knowledge of home health compliance and documentation
  • Excellent interpersonal skills and a commitment to patient-centered care

Responsibilities:

  • Provide skilled nursing visits, develop and implement individualized care plans
  • Administer medications, perform wound care, and manage specialized therapies as ordered
  • Monitor patient status and promptly report changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease management and self-care practices
  • Accurately document assessments and care in HCHB
  • Collaborate closely with the healthcare team to ensure coordinated care
  • Maintain rigorous infection control and uphold patient safety at all times
  • Engage in case conferences, quality improvement, and required trainings
