Job Description
A rewarding opportunity is available for an experienced and empathetic Licensed Practical Nurse (LPN) skilled in home health to deliver exceptional patient care in a field-based setting. This contract position centers on providing nursing services within patients homes, ensuring each individual receives high-quality, compassionate, and compliant care.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility required for patient needs)
- Approximately 2532 points per week (35 patient visits per day)
- Field-based role serving patients within an assigned territory
Key Responsibilities:
- Provide skilled nursing visits, develop and implement personalized care plans
- Administer medications, perform wound care, and manage IV therapy as prescribed
- Monitor and promptly report patient condition changes to the RN Case Manager or physician
- Educate patients and caregivers about disease management and self-care strategies
- Complete timely and accurate documentation using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary care team to maintain plan of care compliance
- Uphold infection control and safety standards in all patient environments
- Attend case conferences, contribute to quality improvement activities, and participate in required training
Qualifications:
- Current and unrestricted LPN license in the state of practice
- Minimum of 1 year of professional experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) required
- Valid drivers license and a reliable, insured vehicle
- Current BLS certification
- Ability to work independently, manage a diverse caseload, and maintain clear communication
Preferred Experience:
- Background in Medicare-certified home health agencies
- Solid understanding of compliance and documentation requirements
- Patient-centered care philosophy and strong interpersonal skills
