A rewarding opportunity is available for an experienced and empathetic Licensed Practical Nurse (LPN) skilled in home health to deliver exceptional patient care in a field-based setting. This contract position centers on providing nursing services within patients homes, ensuring each individual receives high-quality, compassionate, and compliant care.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility required for patient needs)
• Approximately 2532 points per week (35 patient visits per day)
• Field-based role serving patients within an assigned territory

Key Responsibilities:

• Provide skilled nursing visits, develop and implement personalized care plans
• Administer medications, perform wound care, and manage IV therapy as prescribed
• Monitor and promptly report patient condition changes to the RN Case Manager or physician
• Educate patients and caregivers about disease management and self-care strategies
• Complete timely and accurate documentation using Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary care team to maintain plan of care compliance
• Uphold infection control and safety standards in all patient environments
• Attend case conferences, contribute to quality improvement activities, and participate in required training

Qualifications:

• Current and unrestricted LPN license in the state of practice
• Minimum of 1 year of professional experience in home health, hospice, or community-based care
• Proficiency with Home Care Home Base (HCHB) required
• Valid drivers license and a reliable, insured vehicle
• Current BLS certification
• Ability to work independently, manage a diverse caseload, and maintain clear communication

Preferred Experience:

• Background in Medicare-certified home health agencies
• Solid understanding of compliance and documentation requirements
• Patient-centered care philosophy and strong interpersonal skills
