Job Description
Experienced Home Health Licensed Practical Nurse (LPN) needed to provide patient-centered nursing care within a home setting. This full-time contract position requires managing a caseload of patients in their residences throughout an assigned territory, focusing on promoting patient independence, education, and recovery. Strong communication with patients, families, and the healthcare team is essential to ensure optimal care and adherence to care plans.
Location: Crestwood, KY
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM (with occasional flexibility based on patient needs)
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions
- Monitor and report changes in patients conditions promptly
- Educate patients and caregivers on disease processes, medication management, and self-care
- Accurately document all care and communication using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to coordinate patient care and ensure compliance
- Maintain infection control standards and patient safety
- Participate in case conferences, quality improvement, and training activities
Qualifications and Experience:
- Active and unrestricted LPN license in state of practice
- Minimum one year of experience in home health, hospice, or related community-based care
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license with reliable transportation and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth understanding of home health compliance and documentation
- Excellent interpersonal skills with a patient-focused approach
