Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in a home health environment
- Proficient in HomeCare HomeBase (HCHB) EMR system
- Strong assessment and clinical documentation abilities
- Dependable transportation for client visits
- Excellent time-management and organizational skills
- Ability to work autonomously and effectively manage a patient caseload
Key Responsibilities:
- Deliver skilled nursing care and personalized support in accordance with patients care plans
- Monitor, assess, and document patient progress, updating care plans as necessary
- Administer medications and perform treatments following current state standards
- Educate patients and their families on chronic disease management and health maintenance
- Maintain accurate, up-to-date records using HomeCare HomeBase (HCHB) EMR software
- Collaborate with registered nurses, therapists, and other health professionals to coordinate comprehensive care
