An opportunity is available for a Licensed Practical Nurse (LPN) with strong experience in home health care, focusing primarily on geriatric patients. This contract assignment offers the chance to make a meaningful impact through individualized, high-quality care directly in patients homes. Candidates proficient in HomeCare HomeBase (HCHB) EMR and accustomed to maintaining meticulous documentation will be best suited for this position.
Key Responsibilities:
- Deliver skilled nursing interventions to home-based patients according to their individualized care plans
- Monitor and document patient progress, updating plans of care as patient needs evolve
- Administer prescribed medications and perform necessary treatments in accordance with state regulations
- Educate patients and their families about ongoing disease management and prevention strategies
- Maintain accurate, up-to-date clinical records using the HomeCare HomeBase (HCHB) EMR system
- Collaborate with registered nurses, therapists, and other healthcare professionals to provide comprehensive patient care
Schedule:
- Contract assignment with opportunities for extension based on performance and facility needs
Qualifications:
- Current, active LPN license
- At least one year of recent home health nursing experience
- Strong proficiency with HomeCare HomeBase (HCHB) EMR
- Proven clinical assessment skills and ability to deliver exceptional patient documentation
- Reliable transportation required for home visits
- Effective self-management of caseload and ability to work independently
