Deliver compassionate and skilled nursing care to geriatric patients in a home health environment as a Licensed Practical Nurse (LPN). Key Qualifications:
- Active LPN license in good standing
- At least one year of recent experience in home health settings
- Proficient in HomeCare HomeBase (HCHB) EMR system
- Solid clinical assessment, documentation, and communication skills
- Demonstrated ability to work autonomously and manage caseloads effectively
- Reliable transportation for in-home patient visits
Schedule:
- Contract-based; specific duration discussed upon interview
Primary Responsibilities:
- Deliver skilled nursing care to patients in their homes in accordance with individualized care plans
- Administer medications and execute treatments following all state guidelines
- Continuously monitor patient condition, updating care plans and communicating changes as needed
- Educate patients and families on disease management, medication adherence, and preventive health practices
- Accurately document all care and patient interactions using the HomeCare HomeBase (HCHB) EMR system
- Collaborate with registered nurses, therapists, and other care team members to ensure holistic, patient-centered care
