Key Qualifications:

• Active LPN license
• At least one year of recent home health experience
• Proficiency with HomeCare HomeBase (HCHB) EMR
• Outstanding clinical assessment and documentation abilities
• Dependable transportation for traveling to patient homes
• Excellent organizational skills and the ability to work both independently and collaboratively

Primary Responsibilities:

• Deliver skilled nursing care in home settings, following detailed care plans
• Monitor patient progress, update care plans, and communicate any changes
• Administer medications and perform necessary treatments within state regulations
• Educate patients and families regarding disease prevention and care management
• Maintain comprehensive, accurate documentation using HCHB EMR
• Collaborate effectively with RNs, therapists, and other health professionals to support optimal patient outcomes
