Deliver high-quality care to geriatric patients in a supportive home health environment as a Licensed Practical Nurse (LPN). This contract assignment is designed for LPNs with a passion for making a difference in the lives of homebound individuals. Candidates will collaborate closely with interdisciplinary teams to ensure exceptional patient outcomes, utilizing their expertise in home health and the HomeCare HomeBase (HCHB) EMR system.
Schedule/Hours:
- Contract position with potential extensions
Required Qualifications:
- Active LPN license
- At least one year of recent experience in home health
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Solid clinical assessment and documentation capabilities
- Reliable transportation for patient home visits
- Ability to work independently and efficiently manage a caseload
Key Responsibilities:
- Deliver skilled nursing care within patient homes, following individualized care plans
- Observe and document patient progress, adjusting care plans as appropriate
- Administer medications and perform treatments in accordance with state guidelines
- Educate patients and their families on managing care and preventing complications
- Consistently maintain accurate records via HomeCare HomeBase (HCHB)
- Work in coordination with RNs, therapists, and other healthcare professionals to deliver comprehensive care
