Job Description

This contract opportunity calls for a dedicated Licensed Practical Nurse skilled in home health to provide compassionate, high-quality care and education to individuals in need.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Flexibility occasionally required to accommodate patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or community-based care experience
- Proficient in Home Care Home Base (HCHB) charting system
- Valid drivers license with reliable transportation and current insurance
- Current Basic Life Support (BLS) certification
- Solid clinical assessment, time management, and communication skills
- Ability to work independently across assigned territories
- Preferred: Experience with Medicare-certified home health agencies; strong knowledge of compliance/documentation standards; patient-centered approach
Your Responsibilities:
- Conduct skilled nursing visits and develop individualized care plans
- Administer medications, manage wound care, and perform other clinical tasks
- Observe and report changes in patient condition to RN or physician
- Educate patients and caregivers on self-management and safe practices
- Accurately document care and communication in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare teams
- Uphold infection control and ensure patient safety
- Participate in meetings, ongoing education, and quality improvement initiatives
