Job Description

This contract opportunity calls for a dedicated Licensed Practical Nurse skilled in home health to provide compassionate, high-quality care and education to individuals in need.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Flexibility occasionally required to accommodate patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least 1 year of home health, hospice, or community-based care experience
  • Proficient in Home Care Home Base (HCHB) charting system
  • Valid drivers license with reliable transportation and current insurance
  • Current Basic Life Support (BLS) certification
  • Solid clinical assessment, time management, and communication skills
  • Ability to work independently across assigned territories
  • Preferred: Experience with Medicare-certified home health agencies; strong knowledge of compliance/documentation standards; patient-centered approach

Your Responsibilities:

  • Conduct skilled nursing visits and develop individualized care plans
  • Administer medications, manage wound care, and perform other clinical tasks
  • Observe and report changes in patient condition to RN or physician
  • Educate patients and caregivers on self-management and safe practices
  • Accurately document care and communication in Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary healthcare teams
  • Uphold infection control and ensure patient safety
  • Participate in meetings, ongoing education, and quality improvement initiatives
