Job Description
Deliver high-quality, patient-centered care to individuals in their homes as a Licensed Practical Nurse specializing in home health services. This contract opportunity is ideal for nurses with a strong clinical background and expertise in Home Care Home Base (HCHB). As a field nurse, you'll have the chance to foster patient independence and recovery through individualized education, skilled interventions, and clear communication with patients, families, and interdisciplinary care teams.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility may be required to meet patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or similar community-based setting
- Proficient with Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable transportation, and current vehicle insurance
- Current BLS certification
- Excellent clinical judgment, time management, and communication skills
- Ability to work independently in the field and manage a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth understanding of home health compliance and documentation requirements
- Patient-focused and collaborative interpersonal style
Role Responsibilities:
- Provide skilled nursing visits for patient assessments, medication administration, wound care, IV therapy, and interventions as ordered
- Monitor and report changes in patient condition to RN Case Manager or physician
- Educate patients and caregivers on conditions, medication, and self-care
- Complete thorough documentation in HCHB, following compliance and quality standards
- Collaborate with interdisciplinary teams and participate in case conferences and quality activities
- Uphold infection control practices and ensure patient safety at all times
