This contract position provides an opportunity to leverage your clinical expertise while empowering patients to achieve their best health in the comfort of their own homes. Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility for patient needs
• 2532 points per week (approximately 35 patient visits per day)

Key Responsibilities:

• Conduct skilled nursing assessments and develop individualized care plans for home health patients
• Administer medications, wound care, IV therapy, and other clinical interventions as ordered
• Monitor patient conditions closely and report significant changes to the RN Case Manager or physician
• Educate patients and families on medication management, disease processes, and self-care
• Complete thorough, timely documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members for effective care coordination
• Maintain infection control and patient safety standards
• Participate in team meetings, case conferences, and quality improvement initiatives

Required Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least 1 year of experience in home health, hospice, or a similar community-based setting
• Proficiency with Home Care Home Base (HCHB)
• Valid drivers license, reliable transportation, and current auto insurance
• Current BLS certification
• Excellent clinical judgment, organization, and communication skills
• Confident working independently in the field with a dynamic caseload

Preferred:

• Medicare-certified agency experience
• Strong grasp of home health compliance and documentation

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
