Job Description
Make a meaningful impact on patient independence and recovery as a Licensed Practical Nurse providing home health care services. In this contract role, youll use your clinical expertise and compassionate approach to deliver skilled nursing care to individuals within their home environments. You will play a vital part in educating patients and families, promoting positive outcomes, and maintaining the highest quality standards.
Schedule & Setting:
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility to meet patient needs
- Field-based position caring for patients in their homes within an assigned territory
- Caseload typically includes 2532 points weekly (approximately 35 patient visits per day)
- Use of Home Care Home Base (HCHB) required
Key Responsibilities:
- Perform skilled nursing assessments, develop and implement patient-specific care plans
- Provide wound care, medication administration, IV therapy, and other clinical interventions as ordered
- Monitor and report patient status changes to RN Case Manager or physician promptly
- Educate patients and caregivers about disease management and self-care
- Accurate documentation of nursing care and communication in HCHB
- Collaborate with the interdisciplinary health team and participate in case conferences
- Ensure home health compliance, infection control, and patient safety
Required Qualifications:
- Active, unrestricted LPN license in West Virginia
- At least 1 year of home health, hospice, or community-based nursing experience
- Proficiency with Home Care Home Base (HCHB)
- Current BLS certification, valid drivers license, reliable vehicle, and insurance
- Strong judgment, organizational, and communication abilities
- Ability to work well both independently and as part of a team
Preferred Experience:
- Prior work with Medicare-certified home health agencies
- Strong understanding of regulatory documentation and compliance
