Job Description
Deliver compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) by providing expert clinical services in patients homes within an assigned territory. This full-time contract role involves managing a dynamic caseload of 35 patients daily, focusing on promoting patient independence and recovery while maintaining clear communication among patients, families, and the interdisciplinary healthcare team.
Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, perform wound care, and manage IV therapy as ordered
- Monitor patient condition changes and report promptly to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication administration, and self-care
- Accurately document all care and communication using Home Care Home Base (HCHB)
- Collaborate with healthcare team members to ensure care plan compliance
- Adhere to infection control protocols and ensure patient safety
- Participate in case conferences, quality improvement initiatives, and required trainings
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficient in the Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to independently manage a field-based caseload
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused approach
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM (some flexibility for patient needs)
