Job Description

Deliver compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) by providing expert clinical services in patients homes within an assigned territory. This full-time contract role involves managing a dynamic caseload of 35 patients daily, focusing on promoting patient independence and recovery while maintaining clear communication among patients, families, and the interdisciplinary healthcare team.

Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care plans
• Administer medications, perform wound care, and manage IV therapy as ordered
• Monitor patient condition changes and report promptly to RN Case Manager or physician
• Educate patients and caregivers on disease management, medication administration, and self-care
• Accurately document all care and communication using Home Care Home Base (HCHB)
• Collaborate with healthcare team members to ensure care plan compliance
• Adhere to infection control protocols and ensure patient safety
• Participate in case conferences, quality improvement initiatives, and required trainings

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum 1 year of experience in home health, hospice, or community-based care
• Proficient in the Home Care Home Base (HCHB) charting system
• Valid drivers license, reliable vehicle, and current insurance
• Current Basic Life Support (BLS) certification
• Strong clinical judgment, time management, and communication skills
• Ability to independently manage a field-based caseload

Preferred Experience:

• Previous work with Medicare-certified home health agencies
• In-depth knowledge of home health compliance and documentation standards
• Excellent interpersonal skills with a patient-focused approach

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM (some flexibility for patient needs)
