An excellent opportunity awaits an experienced and compassionate Licensed Practical Nurse (LPN) with home health expertise to make a meaningful impact providing patient-centered care in the community. This full-time, field-based contract role focuses on serving home health patients, supporting independence, promoting education, and collaborating with both families and multidisciplinary teams.
Schedule & Hours:
- Monday Friday, 8:00 AM 5:00 PM with occasional flexibility
- Manage 2532 points per week (generally 35 patient visits daily)
- Field setting within your assigned territory
Key Responsibilities:
- Conduct skilled nursing visits and develop individualized care plans for home health patients
- Administer medications, manage IVs, perform wound care, and deliver clinical interventions as prescribed
- Monitor changes in patient health, documenting and communicating promptly with RN Case Managers or physicians
- Educate patients and caregivers on treatment regimens, disease processes, and self-care strategies
- Maintain meticulous documentation utilizing Home Care Home Base (HCHB)
- Collaborate closely with an interdisciplinary team to coordinate and optimize patient care
- Uphold infection control standards, patient safety, and regulatory compliance at every visit
- Attend case conferences, participate in ongoing training, and support quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license for the state of practice
- At least 1 year of recent home health, hospice, or community-based patient care experience
- Confidence and proficiency with Home Care Home Base (HCHB) documentation
- Valid drivers license, reliable transportation, and current auto insurance
- Current BLS certification
- Strong communication skills, clinical judgment, and ability to work autonomously in a field setting
- Preferred: Experience with Medicare-certified agencies and a deep understanding of home health compliance
