This contract opportunity offers an engaging, field-based schedule where youll impact lives while collaborating with a supportive healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (some flexibility for patient needs)

Key Responsibilities:

• Conduct skilled nursing visits and assess, plan, and implement individualized care for patients in their homes
• Administer medications, provide wound care, and manage clinical interventions such as IV therapy per physician orders
• Monitor patient conditions and promptly communicate changes to the RN Case Manager or physician
• Teach patients and caregivers about disease management, medications, and self-care best practices
• Complete accurate documentation and care records using Home Care Home Base (HCHB)
• Maintain active communication with the interdisciplinary care team to ensure patient needs are met and proper protocols are followed
• Consistently uphold infection control and safety standards
• Participate in case conferences, quality improvement activities, and required ongoing training

Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least one year of recent home health, hospice, or community-based clinical experience
• Strong command of Home Care Home Base (HCHB) required
• Valid drivers license, reliable vehicle, and current auto insurance
• Current BLS certification
• Demonstrated clinical judgment, organizational skills, and clear communication
• Ability to work independently and effectively manage a field-based caseload

Preferred:

• Experience in Medicare-certified home health
• In-depth knowledge of home health compliance and regulatory documentation expectations
• Patient-focused and collaborative with outstanding interpersonal skills

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
